Will you verify my benefits?
As a courtesy, Gynecology and Obstetrics of DeKalb, P.C. verifies benefits prior to rendering office or hospital procedures, however there is no guarantee that your insurance carrier will pay your medical claim. Verbal benefits provided by insurance carriers are NOT a guarantee of payment.
May I request a copy of my Medical Records?
All of your medical records are confidential and no information is provided over the telephone without your written consent. Your written authorization is required before your records will be released to insurance companies or others. If you request a copy of your medical records, there is a nominal processing fee. You should receive your records within 5 business days.

How do I file a request for family medical leave (FMLA) or disability-related leave?
If you are eligible for family medical leave or disability leave, your employer or disability carrier will provide you with the proper forms. You will need to complete the form(s) first and then click on the link below to start the process. We do not provide forms, so please make sure you have them before you log on. There is a processing fee and FMLA and Disability forms cannot be completed on the same request. You should receive the completed form(s) in 5 business days.
What if I miss an appointment?
If you are unable to keep your appointment, please reschedule at least 24 hours in advance. Gynecology and Obstetrics of DeKalb, P.C. reserves the right to charge a missed appointment fee if an appointment is cancelled in less than 24 hours.
What are your office hours?
Patient appointments are scheduled from 8:00 a.m. to 4:15 p.m., Monday through Thursday, and Friday from 8:00 a.m. to 1 p.m. Our staff takes pride in seeing patients promptly but due to the nature of our services, unforeseen delays occasionally occur. The receptionist will inform you if your doctor is delayed by a surgery or emergency and make accommodations that are acceptable to your schedule. If you are more than 15 minutes late for your appointment you may be required to reschedule.
What is your ultrasound policy?
It is our practice's policy to schedule a GYN ultrasound after your initial pregnancy consult. This ultrasound will confirm the viability of your pregnancy. The GYN ultrasound will not be considered as part of your global pregnancy benefit and your insurance carrier may designate financial responsibility based on your individual benefits.
If you are scheduled for an ultrasound, you must arrive prior to your appointment. If you are late for your appointment, you will have to reschedule. There are no exceptions.
How can I have my prescription refilled?
Prescription refills requests can be made during office hours only when your medical record is accessible. Most likely, you will be required to leave a voicemail on our Nurse Line that is monitored throughout the day. You will need to provide your full name, date of birth, name of your prescription and pharmacy phone number, and calls will be returned in order of medical need. Please allow 24 hours for prescription refills.
Do you provide estimates for surgery?
As a courtesy, Gynecology and Obstetrics of DeKalb, P.C. will calculate an estimate of patient financial responsibility based on your insurance benefits. This is an estimate only and is required prior to your scheduled surgery.
Do you file claims to a tertiary insurance?
Gynecology & Obstetrics of DeKalb will not file claims to a tertiary insurance however, as a courtesy, our office will provide claims, upon request, for the patient to submit.
