Office Hours / Appointments
Patient appointments are scheduled between the hours of 8am and 4:15pm, Monday through Friday. Our staff takes pride in seeing patients promptly by appointment but due to the nature of our services, unforeseen delays occasionally may occur. The receptionist will inform you if your doctor is delayed by a surgery or emergency and make accommodations that are acceptable to your schedule. If you are more than 15 minutes late for your appointment arrival time you may have to reschedule.
Prescription refills are filled during office hours only when your medical record is accessible. Most likely, you will be required to leave a voicemail on our Nurse Line that is monitored continually throughout the day with calls returned in order of medical need. Please allow the office 24 hours for prescription refills. You will need to provide your full name, date of birth, name of your prescription and pharmacy phone number.
Below are common documents that our practice has made available for our patients. We encourage patients to download, review, and complete these forms before your appointment. By doing so, your initial visit will be expedited since the paperwork will have been completed.
- Notice of Privacy Practices
- Patient Notice of Protected Health Information Acknowledgement
- Patient Information Form
- Health Questionnaire
- Information Packet for New Obstetrical Patients / Expecting Mothers
- Patient Financial & Insurance Policy

Fees and Insurance
We will expect payment at the time of service in full or as designated by your insurance plan. As a convenience for you, we accept payment by cash, check, MasterCard, Visa, Discover or Debit Card as payment options.
If you are insured by a managed care plan, our office will verify benefits and file your claims. Your most current insurance card must be presented at each visit. If you have not satisfied your deductible, or a co-pay is part of your insurance contract, you will be asked to pay this at the time of your visit.
Insurance Plan Participation
Our goal is to participate with as many of the most popular and common managed care and insurance plans as possible. Therefore, our participation in plans are regularly undergoing review and changes. We recommend that you contact our practice or your insurance company to verify our practice's participation in your insurance plan.
